Positive Ageing Support / Regional Assessment Services Officer

Band 6 Level A, Limited tenure – 15 months, Part-time 0.6 – Based in Warracknabeal

Yarriambiack Shire Council is seeking applications for the above Limited Tenure Part-time position from reliable and enthusiastic persons interested in providing efficient operation and delivery of community care services and providing a Home Support Assessment for people referred to My Aged Care Commonwealth Home Support Program, focusing on promoting autonomy and independence to enhance their wellbeing and quality of life.

Key objectives include:

  • Providing efficient operation and delivery of high quality community care services
  • Assisting with the compliance of all community care and volunteer services
  • Ensuring that services are client focused, flexible and responsive to the needs of eligible residents and delivered in a manner which maximizes their independence and ability to remain in their community
  • Developing Support Plans which identifies the concerns, goals, and preferences of the consumer with a focus on wellness, reablement and restorative care on a short term basis, or of an ongoing nature to maximize independence.

This position is classified as Band 6, Level A with a pay rate of $41.8400 per hour.
