Are you passionate about making a difference to the lives of people over 65 and people eligible for the HACC under 65 program. Are you looking to enhance and maintain the capacity of people and assist them to achieve their goals?

The Yarriambiack Shire Council has an exciting opportunity for an enthusiastic and skilled allied health or nursing professional to undertake assessments of clients in their homes across the Shire.

The CHSP and HACC programs provides a range of services to frail, vulnerable and isolated older persons, people with a disability, and their carers, who wish to continue living at home with supports to maintain independence.

The role of the Assessment (LAHA) Officer will include coordinating day to day functions of Intake and Assessment services and undertaking holistic living at home assessments for clients.

The successful applicant will have a strong understanding and experience in the implementation of the Wellness and Reablement with clients in problem solving and developing goal centred care plans. The applicant will have a good understanding of continuous improvement and excellent written and verbal communication skills.

The Applicant will have an awareness of CHSP/HACC Policy, Best Practice, Industry Developments and Legislation.

A tertiary qualification in allied health or nursing is required as well as demonstrated experience in working with the frail aged or persons with a disability.
